A Clinical and Radiological Study of the Mandibular Condyle Fractures in Adults

Abstract


The aim of this study was to evaluate the healing course of mandibular condyle fracture and to determine the most suitable treatment method. We conducted a retrospective analysis of adult 36 mandibular condyle fractures among 26 patients who were treated in Department of Oral and Maxillofacial Surgery, Gyeongsang National University Hospital. We evaluated the preoperative clinicoradiological variables and postoperative results and outcomes. In 36 condylar fractures (26 patients), there were 11 cases (30.5%) of condylar head, 14 cases (39.0%) of condylar neck, and 11 cases (30.5%) of subcondylar fractures. The results were as follows ; (1) All condylar head fractures were treated by conservative method. After initial treatment, we could see one case of mouth opening limitation and the other case of anterior open bite. (2) In the cases of condylar neck fractures, 9 cases were treated by conservative method and 5 cases were treated by surgical method. We could see one case of anterior open bite in the conservative treated cases and other one case of postoperatively infection in the surgical treated cases. (3) In the cases of subcondylar fractures, 2 cases were treated by non-surgical method, the other 9 cases were treated by open reduction method. In these cases, we could not find any specific complication. (4) We could not obtain complete segmental reduction with close reduction technique in 17 cases (11 cases of condylar head and 6 cases of condylar neck fractures), those cases showed condylar morphologic changes in routine radiogram during follow-up period. We concluded that the fracture sites were the most effective factor to determine the treatment method inthe adult mandibular condyle fractures and we prefer to the surgical method to reduce patient¡¯s complaints in the subcondylar fractures. But, in the fractures of condylar neck or head, it is better to consider various factors that influence on treatment methods and prognosis ; fracture level, displacement of segments, medical problems, scar formation, status of dentition, and concomitant injuries.
